Over 15 years, the thousands of teams and tens of thousands of participants in our programs have been our laboratory. Our job has been to engage, mobilize, and create impact - at scale. Our teams have been large, small, rural, urban, safety net, and community-based teams:
Teams that had executive leadership (sometimes), and teams who didn't (often).
Teams with dedicated resources (rarely) and teams that didn't (mostly).
Teams with cross-continuum partnerships (rarely) and teams that didn't (mostly).
Teams with enabling technology (rarely) and teams that didn't (mostly).
We have used this laboratory to identify the best of what works, most often, for most teams, in the widest variety of settings. And, we leave behind the seemingly good or popular ideas that didn't work, well enough, or often enough. The result is a series of methods to transform care to reduce recurrent acute care - at the state level, the population level, and for the highest risk groups - by implementing whole-person, team-based, cross-continuum care that is data-informed, clinically credible, operationally feasible - and effective. The impact of our work is the quintuple aim: equitable, high quality care, transformed lives, improved professional satisfaction and clear evidence of impact for staff, and measurable impact on healthcare utilization and cost.
